2–3 hour session: weighted feed, physical exam, education, personalized care plan
Receive invoice, insurance claim letter, and claim form for reimbursement submission

DOD ID or official documentation required. Note: Military contractors are not eligible for this discount.

Out-of-network provider. Clients receive full documentation to submit to insurance for possible reimbursement.
